Am I a good candidate

If you are 18 year-old or older and if you are not planning to become pregnant very soon or not breast feeding at least for the last 9 months, then it may be worthwhile to book your consultation with Dr Mounir so he can assess you and discuss with you why you are not happy with the size of your breasts. Please bring with you a copy of your NHS medical records to show that you are healthy physically and mentally.


What is the right size implant for me

Well if you are interested in breast implant you should have done some research by now and probably you have already heard of the rice test. Please keep in mind that the rice test is not a precise test and it is not recognised by any medical body. However it still does provides a rough idea about the volume that you would like to achieve with breast implant.

However when you are in consultation with Dr Mounir and after he has examined your breasts, based on your anatomy and your frame, Dr. Mounir will help you to choose the right implant by offering you the MENTOR Volume Sizing System for Breast enhancement.

Effectively, the Sizing System contains a set of 18 shaped volume seizers and so you will be able to simulate different breast implants volumes, starting from 150cc to 655cc, and so you feel very proactive and involved in choosing your implant size.

Should I choose anatomical or round implants

Dr Mounir will help you to choose between anatomical implants or so-called teardrop implant and round implant. First, he will explain the volumes on anatomical and round implants are not identical. Effectively, implants with almost similar width and width, anatomical implants are about 20% lighter than round implants.

It is a fact that anatomical implant will produce a more natural look where rounded implants will produce more fullness at the top.

Round implants have been in use since 1963 and they still remain the most popular used implant for breast cosmetic surgery in UK.

If used above the chest muscle without adequate breast-tissue cover, they will produce a fake rounded look. However, when placed behind the chest muscle using a dual plane partial technique, round implants can actually produce a natural looking and very good results.

What are the risks of breast augmentation

Bleeding 1%

Bleeding could occur up to two week after the surgery. If you do notice you’re your becomes swollen and tight, then you need to contact Dr Mounir urgently. He will probably take you back to the surgery room and evacuate the Heamatoma and stop the bleeding.

Infection (less than 1%)

This is very serious complication that could happen up to 4 weeks after the surgery. If you do notice that your breast is swollen, red and painful, you will need to contact Dr Mounir urgently. He will assess you and depending on the nature and the extend of the infection he will either prescribe for you some antibiotics or he will take you back to the surgery to remove the infected implant. Unfortunately you will be left without implant for at least 3 months.

Capsular contracture (10%)

In some patients the breasts become very hard and source of concern to them as it can cause some discomfort. If this happen you should contact Dr Mounir to discuss the situation. Based on his finding and if the breast is not too hard, he may advise to wait and see. However, if you are experiencing some discomfort then Dr Mounir may advise to have the implant removed and the scars tissue release (capsulotomy) or even a removed (Capsulectomy). Please be aware that Capsular contracture is the most common complication that need re-operating and revision surgery.

Less feeling in the breasts

Some women may experience oversensitivity of the nipples. Other may experience hyposensitivity (less feeling). The truth is that every patient heals differently and these symptoms do settle down with time but unfortunately they could be permanent.

Rippling or palpability of the implant

During your consultation with Dr Mounir, he will direct you to read more about the risk of palpability and rippling on the British association of plastic and reconstructive surgeons (BAPRAS). You will learn that this is a very commun complication that is almost inevitable in patient with flat chest (slim patients). There is no implant rippling free and there is no surgeon who would be able to guarantee you that you will be 100 % rippling free.

Implant failure

Usually Dr Mounir uses Mentor implants. These implants are made with a very solid and tough shell. However if you experience some changes in the size or shape please contact Dr Mounir immediately and will most probably refer you to have a scan to confirm or not a rupture of implant.


What I should choose Textured vs Smooth implants?

The difference between smooth and textured implant is that the later has a rough surface. The idea was to make the surface of the implant connect to the tissue that surrounds them, preventing them to move inside the breast pocket. This is very important when you are having a teardrop implant because any movement of these anatomical implants will produce a breast deformity.

But these are the facts:

  • Dow Corning Corporation in United States manufactured the first silicone gel breast implant in 1962. These were smooth implants.
  • In 1972, second-generation gel implants was produced to improve the cosmetic outcome and reduce the risk of capsular contracture. It was the first time when we heard of the polyurethane textured surface.
  • However because of the risk of cancer associated with the polyurethane, these implants have been immediately been abandoned and new type of textured surface was produced on the actual silicone polymer shell to copy the effects of polyurethane in order to reduce the risk capsular contracture rates.
  • These nonpolyurethane textured implants became very popular and were used extensively all over the world.
  • Even now surgeons around the world still using textured silicone gel implants because of the fact that the rate of capsular contracture is far less capsular contracture compared to smooth implants.
  • Although the benefits of textured implants include a reduced risk for capsular contracture, recent reports in the literature have shown that the textures implants are associated with text ALCL
  • During your consultation, Dr Mounir will clearly tell you that if you have decided to proceed with your breast augmentation surgery and you wish to avoid the risk of ALCL (Anaplastic Large Cell Lymphoma) based on the data that we have today his recommendation would be to have the smooth implants.

Should I have my implant over or under the muscle

Over the muscle

The insertion of implants over the chest muscle is supposed to be a straightforward and the safest technique for breast enhancement. The recovery time is quicker and the discomfort is minimal. However this option is mainly indicated for patient with enough breast tissue to avoid risk of rippling and palpability.

Behind the muscle

If you are slim and you do not have a lot of breast tissue to cover the implant, then placing your implant under your chest muscle is a very good option. This will reduce the risk of rippling. The sub-muscular implantation is also called sometimes “dual plane” or “partial” breast augmentation. This technique is however associated with the risk of animation deformity and double fold.
